The request for arbitration must include the identifying particulars of the parties to the arbitration, a brief statement of the contractual relationship, the arbitration agreement, the subject matter of the dispute and its facts, a summary of the applicant's claims, in addition to a proposal for the appointment of the arbitrator or a notification thereof, in accordance with Article (9) of the Executive Regulations of the Arbitration Law.
